Preparing for your appointment, please read the following information as it is very important and will help speed up your visit:

• Expect your doctor appointment to take anywhere from 30 minutes to an hour depending on whether you are having an evaluation done by one of our practitioners.  Evaluations are required when you don’t have the necessary documentation that the doctor needs in order to evaluate whether you would benefit from the use of Medical Marijuana (MMJ).
• Upon arriving you will be filling out a series of paperwork which will take approximately 15 minutes which include your application, HIPPA form, and any pertinent waivers (soon to be download-able on this website).

Primary Medical Marijuana Center Info (used to be Caregiver Info):

You will be given the choice as to whether you are going to name CPM as your Primary Medical Marijuana Center or not.  When naming us (or anyone) as your Primary MMC, what that allows the person or party to do is both possess two ounces or grow six plants ‘in your name’. When checking off the boxes on your application, you will notice that you can either name CPM as your MMC or name both yourself and CPM, giving us the ability to serve you while you are also in the process of growing your own MMJ.

If we are your Primary MMC, you will receive a 10% discount off of everything including massge and acupuncture (and all other therapies).  Naming us as your MMC allows us to grow more medicine “in-house” lowering our cost which is passed along to you, the patient.  As a community working together, the more patients that name us as their Primary MMC, the better off we all are.
